Service checklist template

I'm servicing my grandmas car for her tomorrow, she has asked if I could give her a check list of things that have been done, so she can put it with her records when she decides to sell the car

Does anyone have one or know where to get one from that doesn't say sample across it

It's not for professional use or anything just for her records

Just a handwritten page should suffice. You're not pretending that a garage did the service, so typing up or filling a form seems OTT.

23 July 2016 - 60,000miles
Replaced oil and filter
Checked all lights
Checked tyres for tread and condition
greased door straps
etc
